Social Prescribing

We provide a Social Prescribing Service on behalf of 3 Primary Care Networks (clusters of GP practices) across Redcar and Cleveland: Coastal, East Cleveland and Eston.

Redcar & Cleveland Borough Council provides a Social Prescribing Service on behalf of 3 Primary Care Networks (clusters of GP practices) across Redcar and Cleveland: Coastal, East Cleveland and Eston. 

Referrals come from primary care, self-referral and wider health and social care partners. 

Your local Social Prescribing Service is based in GP practice and is there to support you with non-medical issues that are affecting your wellbeing. 

The Social Prescribing Practitioners will listen to what matters to you and will connect you to services and community activities that may support you to feel happier and healthier. Here are some of the things that people might like to talk about with a Social Prescriber:

  • Feeling lonely/isolated/wanting to make new connections.
  • Feeling down, stressed or anxious
  • Managing health conditions and symptoms
  • Keeping active
  • Benefits, money and housing problems
  • Living with chronic pain
  • Making healthier choices
  • Reducing smoking or alcohol
  • Caring for someone

Appointments are longer than typical GP appointments and can be in the surgery, in the community or over the phone. This gives you time to talk about what matters to you, in a place you feel comfortable.

How do I access the service?  

It’s simple, call your Doctors Surgery and ask to be referred to a Social Prescriber.
